In the intricate world of canine healthcare, the issue of over-vaccination stirs significant debate. While the American Animal Hospital Association (AAHA) advises extending vaccine boosters every three years, some veterinarians cling to the tradition of annual boosters. This discrepancy leaves many dog parents in the dark about the potential risks of excessive immunization.

Vaccines challenge a dog’s immune system profoundly. They can deplete essential nutrients such as vitamins C, D, A, zinc, magnesium, and selenium. For a vaccine to work effectively, a dog must have a robust immune system capable of responding vigorously. Therefore, only dogs in peak health should receive vaccines, not dogs that are fighting illness or about to have a surgical procedure.

The development and functioning of attenuated, inactivated, and recombinant vaccines in dogs involve sophisticated technologies to safely immunize against various diseases. Each type has its specific applications and benefits, tailored to different situations and risks.

Attenuated vaccines for dogs involve live pathogens that have been weakened so they cannot cause severe disease but still replicate to some extent to provoke a robust immune response. These vaccines simulate a natural infection, which helps the immune system develop a long-lasting response. For instance, modified NS1 proteins in canine influenza viruses have been used to create live-attenuated vaccines, offering superior immunogenicity and protection compared to inactivated vaccines.

Inactivated vaccines contain viruses or bacteria that have been killed or inactivated. These vaccines are safer for immunocompromised or very young animals as they cannot revert to a pathogenic form. However, they might induce a weaker immune response than live vaccines, often requiring adjuvants to enhance their efficacy. An example is UV-inactivated recombinant plant viruses that protect dogs from canine parvovirus, showing that even non-replicating entities can effectively stimulate the immune system.

Adjuvants are substances added to enhance the immune response to the vaccine, promoting more robust and durable immunity against diseases. These adjuvants function in several ways: they intensify the immune response to the vaccine's antigen, extend the period of immunity to reduce the frequency of boosters, and enable the use of smaller antigen doses for adequate immunization. Common adjuvants in veterinary vaccines include aluminum salts, like aluminum hydroxide or aluminum phosphate, and emulsions, each selected based on the vaccine’s specific needs and how they interact with the immune system.

Although adjuvants are essential for maximizing vaccine effectiveness, common adverse effects include reactions at the injection site—such as pain, swelling, redness, and hardness—that usually resolve within a few days. Animals may also experience mild fever and lethargy, signs that the immune system responds to the vaccine. More rare complications include allergic reactions, which might present as hives, facial or limb swelling, breathing difficulties, or severe itching. Additionally, severe local reactions can lead to the formation of granulomas or abscesses at the injection site, requiring longer healing times and possibly veterinary intervention.

Recombinant vaccines utilize genetic engineering techniques to produce a safe vaccine that exposes the dog's immune system to a part of the virus or bacterium, typically a protein, to build an immune response without the risk of causing the disease. For instance, a recombinant canine distemper virus has been engineered to express the rabies virus glycoprotein, providing dual protection against both rabies and canine distemper.

These vaccine types are crucial for managing health in canine populations, effectively preventing the spread of contagious diseases while maintaining safety and efficacy. 

In veterinary medicine, vaccines for dogs are categorized into "core" and "non-core" vaccines based on the essential need and the specific risks that various environments pose to the dog.

The core vaccinations are widely accepted, as most people understand the necessity of essential vaccines against canine distemper (CDV), canine parvovirus (CPV), canine adenovirus (CAV), and canine parainfluenza to safeguard our dogs from life-threatening diseases that are globally prevalent. These diseases have high fatality rates, and vaccination offers significant protection against them. Basic immunizations are crucial for every puppy but should only be initiated once the maternal immunity has waned around 9-12 weeks of age.

Most veterinarians will encourage you to give all core vaccines in an all-in-one dose vaccination, starting at a very early age (6 weeks) and revaccinating at 2-4 week intervals until the pup is 16 weeks of age. However, providing so many immunizations simultaneously is challenging to your puppy’s immune system. 

Non-core vaccines are administered based on a dog's specific risk of exposure to particular diseases, such as Leptospirosis, Lyme disease (Borrelia burgdorferi), Bordetella bronchiseptica (infectious tracheobronchitis and, occasionally, pneumonia), and Canine influenza virus (H3N8). The decision to use these vaccines should be tailored to the individual dog's lifestyle, geographic location, and other risk factors.

Vaccinosis or Vaccine-Induced Disease is a set of adverse reactions that can occur after a dog is vaccinated. While generally rare, these reactions can range from mild, transient, severe, and potentially life-threatening. 

Vaccinosis represents a complex and poorly understood set of conditions that can occur after canine vaccinations. While vaccinations are crucial for preventing severe diseases in dogs, they can also lead to adverse reactions in a small number of cases. The mechanism behind vaccinosis is not fully understood. Still, it is believed to involve an inappropriate immune response triggered by the vaccine's components. It leads to an autoimmune reaction where the immune system mistakenly attacks its cells and tissues.

Vaccinosis can include immediate allergic reactions, longer-term immune-mediated conditions, and other health issues potentially linked to vaccines. An example of a severe vaccine-induced condition is immune-mediated hemolytic anemia (IMHA), which has been reported to occur in dogs following vaccination. In a controlled study, a higher frequency of IMHA was found in dogs recently vaccinated compared to non-vaccinated dogs. Other long-term effects include food allergies, skin health issues, imbalanced thyroid hormones, and cancer.

Incorrect dosage of the vaccine is thought to be a cause of vaccinosis. For example,  the vaccine dose for a Shih Tzu is often the same as that given to a Great Dane. Common sense tells us that the dosage should differ for dogs that differ in size. Add an annual booster of the same dose, and you can see where a small dog can easily be over-vaccinated!

To minimize the risk of vaccinosis, veterinarians might adjust vaccine protocols based on a dog's specific health status, age, breed, and previous vaccine reaction history. A personalized approach can help reduce the incidence of adverse reactions.

Vaccines are generally safe and essential in protecting dogs from life-threatening diseases. They can have potential side effects. Although most dogs experience minimal or no adverse reactions to vaccines, there are potential side effects ranging from mild and short-lived to chronic disease. 

A comprehensive study tracking 1,226,159 dogs identified 4,678 vaccine-associated adverse events (VAAEs), equating to 38.2 events per 10,000 dogs vaccinated. The study highlighted that young adult, small-breed, neutered dogs that received multiple vaccines in a single office visit were the most susceptible to experiencing a VAAE within 72 hours post-vaccination. Recorded adverse events included nonspecific reactions, allergic reactions, urticaria (hives), and anaphylaxis.

Allergic reactions are among the potential side effects, with severity ranging from mild symptoms such as facial swelling, itching, and hives to more severe conditions like diarrhea, vomiting, and, in extreme cases, anaphylaxis. Anaphylaxis is a rare but urgent medical situation that demands immediate attention from a veterinarian, as it can lead to shock and, potentially, death.

Mild localized reactions at the vaccination injection site can occur.  Dogs might experience pain, swelling, or develop a small lump where the vaccine was administered, although these signs typically resolve without intervention. 

Dogs may exhibit mild systemic reactions, such as transient symptoms like lethargy, low-grade fever, or a slight decrease in appetite after vaccination. These clinical signs typically resolve without intervention.

On rare occasions, Vaccine-Associated Sarcoma (VAS) is an aggressive type of cancer that may develop at a previous vaccine injection site. For dogs, the occurrence of sarcomas at vaccine injection sites is not well-documented, and very few cases are reported in veterinary literature. Unlike in cats, where the incidence of VAS is estimated to be between 1 in 1,000 to 1 in 10,000 vaccinated animals, no similar well-defined risk is established for dogs. The rarity of reported cases in dogs suggests that if VAS does occur, it is at a much lower frequency than in cats.

Immune-mediated hemolytic anemia (IMHA) has also been suggested to have a possible link to vaccinations. IMHA in dogs is a severe and potentially life-threatening condition where the dog's immune system mistakenly identifies its red blood cells as foreign invaders and starts destroying them. The autoimmune response leads to hemolytic anemia, characterized by reduced red blood cells, significantly impairing the blood's ability to carry oxygen to tissues throughout the body. If left untreated, IMHA can be fatal. 

The relationship between vaccinations and the development of polyarthritis in dogs has been explored in several studies, but the evidence is mixed and somewhat limited. Although there are documented cases of individual dogs developing polyarthritis post-vaccination, large-scale data do not conclusively support the potential link between vaccinations and polyarthritis. Further research with larger sample sizes and controlled conditions is needed to understand this relationship fully.

Lastly, some dogs may exhibit gastrointestinal or behavioral changes after vaccination. Gastrointestinal clinical signs can include vomiting, a lack of appetite, constipation, or diarrhea. At the same time, behavioral changes can consist of restlessness, listlessness, or a decreased willingness to hunt, especially after consuming oral rabies vaccine baits. 

Nosodes for dogs are offered in homeopathic and holistic pet care as alternatives to traditional vaccines. They are prepared from materials derived from diseases or the products of diseases and are used to prevent or treat various conditions in pets. The concept behind nosodes in homeopathy is based on the principle of "like cures like," where substances that cause clinical signs in a healthy dog are used in very diluted forms to treat similar clinical signs in a sick dog. 

In the preparation of nosodes, the original substance is serially diluted, often to the point where no molecules of the original substance remain in the final product. This dilution process is accompanied by a specific method of shaking known as "succussion," which homeopaths believe imbues the solution with therapeutic properties.

Commonly mentioned nosodes for dogs include those purported to protect against or treat conditions such as:

Parvovirus: A highly contagious viral illness that affects dogs, particularly puppies. A nosode is available that claims to offer protection against this virus.

Distemper: Another serious viral disease affecting dogs, with a nosode claiming to provide an alternative to the conventional vaccine.

Kennel Cough (Bordetella): An infectious bronchitis in dogs that is highly contagious, for which holistic practitioners often use a nosode.

Rabies: A fatal viral disease with a nosode available used by some as an alternative to the legally required rabies vaccination in some jurisdictions.

Leptospirosis: A bacterial disease that affects both animals and humans, with a corresponding nosode claimed to help in prevention.

Using nosodes as a substitute for traditional vaccinations in animals, including dogs, is highly controversial and not supported by mainstream veterinary science. The effectiveness and safety of nosodes have yet to be scientifically validated, and they are generally not recommended by veterinarians. 

Titering is Important in Checking For Immunity

Titer testing is used to determine immunity for various diseases, including but not limited to parvovirus, distemper, adenovirus, and rabies. The primary goal of titer testing is to assess the dog's immune response to previous vaccinations or natural exposure to infectious agents. Titers help determine if the dog has adequate protective immunity against specific diseases. A blood sample is taken from the dog for the titer test. The blood is analyzed in a laboratory to measure the concentration of antibodies against specific pathogens. Antibodies are proteins the dog's immune system produces in response to exposure to a pathogen, such as a virus or a bacterium. Antibodies play a crucial role in neutralizing and fighting off the infectious agent.

The titer test results indicate the level of antibodies present in the dog's blood. Titer tests measure only the humoral immune response (antibodies in the blood). They do not measure the cellular immune response, which can also be crucial in fighting infections. Therefore, a dog with low antibody titers might still have some level of protection due to cellular immunity, though this is not currently measured in routine titer testing. So, if a dog's titer levels show any protective immunity, there may be no need for an immediate booster vaccination. 

Titer testing can be a valuable tool, especially for dogs with uncertain vaccination histories or those with potential adverse reactions to vaccines. It helps avoid over-vaccination while ensuring the dog is protected against infectious diseases.

Dog parents should be encouraged to base their decision to vaccinate their dog on the prevalence of the disease in their area. For example, in Connecticut, Lyme disease (LD), a tick-borne disease, was first characterized in 1975. Cases of LD are found in most continental United States, with the highest prevalence in the Northeastern states and Minnesota and Wisconsin.  For dogs residing or visiting these states, the dog parent may decide that preventing LD overrides the risk of the vaccine side effects. For dogs living in the Western United States, dog parents may determine that the minimal risk of Lyme Disease does not warrant a vaccination.

The need for vaccination for Bordetella, more commonly known as kennel cough, can also be assessed based on risk.  Although kennel cough is highly contagious if a dog is in direct contact with other dogs, it is self-limiting for healthy dogs. If a dog rarely encounters other dogs except on the occasional walk, its risk of getting kennel cough is low.  This may be a vaccine that dog parents forego. 

Only healthy dogs should be vaccinated. In many cases, veterinarians vaccinate dogs that are being seen for medical issues. If the dog is undergoing blood tests, refrain from vaccinating until the results show that the dog is healthy enough to be vaccinated. All the medical problems should be resolved before the dog is vaccinated. Dogs should not be vaccinated right before a surgical procedure to avoid the additional stress on the dog’s body.

Maternally derived antibodies (MDA) in puppies offer initial protection against various infections, but their protective effect wanes as the puppy grows. By 16 weeks, most puppies can mount a robust immune response to vaccines. 

The ideal age to start vaccinations for puppies is typically around 6 to 8 weeks. This schedule is designed to offer protection as early as possible while accounting for the natural immunity puppies receive from their mother's milk, which can interfere with the effectiveness of vaccines. A general guideline for puppy vaccinations:

At 6 to 8 weeks, the first dose of core vaccinations for distemper, parvovirus, and adenovirus (canine hepatitis). Vaccines are available for conditions like Bordetella (kennel cough), depending on the puppy's location and risk factors.
